Holders who pay an owner after remitting property to the State may apply for a reimbursement. To submit a reimbursement, please mail Form COT/ST 917 (pdf) (Holder Claim Form) to our physical address. Claims greater than $1000 must show proof of payment or delivery of the property to the owner. Note, a copy of your W-9 is required for all reimbursements.
